These terms apply between you and Halden Clinic ([registered company name and number]), 14 Linden Avenue. By using this website or booking a visit, you agree to them. Our privacy policy explains how we handle your personal information.
Halden Clinic is an outpatient clinic. We don’t provide emergency care, and the booking form isn’t monitored around the clock. In an emergency, call your local emergency number or go to the nearest emergency department.
The content on this website is general information. It doesn’t replace a consultation with a doctor, and it isn’t a diagnosis or treatment plan for you. Please talk to a clinician before making decisions about your health.
Sending the booking form is a request, not a confirmed appointment. We’ll call you back within one working day to agree a time. Your appointment is confirmed once we’ve sent you the date, time and location in writing.
Please give us at least [24 hours’] notice if you need to cancel or move an appointment, so we can offer the time to another patient. Late cancellations or missed appointments may be charged [a fee of …], unless the reason was outside your control.
We’ll tell you the cost of a visit, test or treatment before it takes place. If you send us your policy number, we’ll check your cover and confirm in writing what your insurer pays. Anything your insurer doesn’t cover is payable by you, and we’ll always tell you before you incur it.
